I understand that by using Pathway Drugs, Incorporated’s (“Pathway”) Services or Platform (as defined in Pathway’s Terms of Service) I have been expressly made aware of and agreed to certain Disclaimers and Consents, including, but without limitation to the following:

2. IMPORTATION OF PRESCRIPTION DRUG FOR PERSONAL USE

I understand that I am ordering this prescription drug directly from a licensed pharmacy outside of the United States, and that I will be importing it into the United States for my direct personal use. The prescription drug will not be distributed by any pharmacy or wholesaler in the United States, and I will not resale or distribute the product.

3. NO AFFILIATION WITH DRUG MANUFACTURER

I understand that the manufacturer of the prescription drug I am ordering and its United States affiliates are not connected with Pathway, are not responsible for this transaction, and the product will be imported without the authorization of the manufacturer.

I understand that obtaining a prescription drug imported from a licensed pharmacy outside of the United States is entirely my choice, and I am not required to import this drug. I may choose at any time to obtain my medication at pharmacies or other licensed providers in the United States.

I understand that this prescription drug may differ from the version sold in the United States in its packaging, labeling, instructions for use, language, units of measurement, warnings, statements, appearance, or other attributes as disclosed in the Disclaimer and Consent Form. The drug is approved, packaged, labeled, and dispensed in accordance with the laws and regulations of the country from where it is ordered. I understand these possible differences, and even though these differences could be relevant to my order decision, I voluntarily choose to order the product from the licensed pharmacy outside of the United States with this information in mind.

6. SUPPLY CHAIN AND QUALITY CONTROL DIFFERENCES

I understand that this prescription drug will be distributed, stored, and supplied in accordance with the laws, regulations, and pharmacy practices of the country from where it is ordered, rather than those used by pharmacies and wholesalers in the United States. The product may be brought into the United States through supply chain methods not authorized or controlled by the manufacturer or their authorized distributors. The recall practices or warranty for this product may be different than for a product sold in the United States. I understand these possible differences, and even though these differences could be relevant to my order decision, I voluntarily choose to order the product from the licensed pharmacy outside of the United States with this information in mind.
